The MVC has started to mail out license renewals with a new form attached.
Bob Leonhardt from the MVC provided the following guidance for completing the
NJMVC CDL Holder Self-Certification Form.
If you drive a school bus the following applies to which category you would check:
Category 1 – check this block if you drive for a Private Company and transport Interstate
Category 2 – check this block if you drive for a BOE and transport Interstate
Category 3 – check this block if you drive for a Private Company and transport Intrastate
Category 4 – check this block if you drive for a BOE and transport Intrastate
Note: NJ requires all holders of a CDL with a “P” endorsement to submit a medical
